New Patients

Once you are scheduled for a test, procedure or surgery, you will be contacted by a representative from our Patient Access Department. The purpose of this call is to verify your demographic and insurance information. Some procedures require insurance prior authorization, which needs to be completed at least 24 hours before your scheduled procedure. If you are scheduled for surgery, you will receive an additional call from a nurse to discuss your procedure prior to your scheduled appointment.

 
Registration

If you are unable to pre-register in advance, allow extra time to complete the registration process when you arrive at the hospital.
On the day of your appointment, please register and /or check–in at one of our outpatient registration desks. When you schedule your appointment, you will be given the location at which you should register. Directions to each are below.

Southlake Diagnostic Center: Monday thru Friday, 7:00 a.m. to 7:00 p.m. and Saturday, 7:00 a.m. to 12:30 p.m.
The Outpatient Diagnostic Center is in the first building on the right as you enter the Southlake campus from Broadway. You may park in any of the lost to the north and west of the building. Enter the building through the main entrance and turn right through the glass doors into the Diagnostic Imaging Center on the first floor. Proceed to the front desk to complete your registration. You will be given directions at that point on where to go for your test.

Southlake Main Hospital - Diagnostic Imaging, EDX: Monday thru Friday, 7:00 a.m. to 4:30 p.m. 
Some diagnostic imaging tests and procedures take place in the main hospital building. In this case you will register at Outpatient Registration, which is located next to the Emergency Department. Enter at the door marked Outpatient Registration and go to the first desk on the left to register. You will be given directions at that point on where to go for your test.

Southlake Surgery Registration – Monday thru Friday, 7:00 a.m. to 4:30 p.m.
To register for surgery at the Southlake campus, please park in the visitor lots just across 89th Street from the main entrance. Enter the hospital through the main entrance and proceed across the lobby to the Surgery Registration desk, which is located across from the cashier window. You will be given directions at that point on where to go for your procedure.

Emergency Department Entrance: Monday thru Friday, 4:30 p.m. to 7:00 a.m.
For tests and procedures conducted after 4:30 pm, you will register at the Emergency Department. Follow the signs that say Emergency and enter though the ED doors to check in at the Triage desk. From there, someone will escort you to the location of your test or procedure.

Northlake Outpatient Registration: Monday thru Friday, 7:00 a.m. - 4:30 p.m.
All patients at the Northlake campus will register at the Outpatient Registration Desk just north of the main lobby and past the gift shop. Enter the main hospital entrance, take the stairs or elevator to the first floor. Pass the information desk and the gift shop to reach the Registration Desk

Emergency Department Entrance: Monday thru Friday, 4:30 p.m. to 7:00 a.m.
For tests and procedures conducted after 4:30 pm, you will register at the Emergency Department. Follow the signs that say Emergency and enter though the ED doors to check in at the Triage desk. From there, someone will escort you to the location of your test or procedure.

 
What to Bring to Registration 

For all services you will be asked to present your picture identification and insurance cards. For your safety, all information will be verified and you will be asked to sign consent forms and receive an armband. 

You should also bring essentials such as a list of your medications (if not recorded during pre-registration), clothes to wear at discharge, glasses and hearing aids. Please remember to leave valuables such as money, credit cards and jewelry at home.
