Community Health Needs Assessment

Developing of a plan of action for the hospital to address the needs and barriers of residents and communities in achieving optimal health.

The Community Health Needs Assessment (CHNA) is a systematic, data driven exploration of the major health problems, gaps in services and other factors that contribute to less-than-optimal health status for residents of the communities in Northwest Indiana served by Methodist Hospitals. The assessment provides guidance in the development of a plan of action for the hospital to address the needs and barriers identified in the study.

The Community Health Needs Assessment is conducted every three years as required by the Patient Protection and Affordable Care Act (PPACA) and Section 501(r)(3) of the Internal Revenue Service Code.

The most recent CHNA was approved by the Methodist Hospitals Board of Directors on December 5, 2022.
