Instructions for completing the Nursing Excellence Award Nomination Form

To nominate a nurse for the Nursing Excellence Award, submit the Nursing Excellence Award Nomination Form with supporting evidence that addresses the criteria for the award. Evidence is required for each of the four (4) criteria listed on this form. Please explain, give examples or scenarios of how your nominee exemplifies Nursing Excellence in each of the categories listed below.

    Serves as a mentor / role model / preceptor / coach on the nursing unit; Participates in unit based Shared Governance activities; Develops unit based educational opportunities or unit based inservices (i.e., poster; flier; bulletin board; and/or handout). Serves as a UBET on the nursing unit; Involved in a Shared Governance Council; Member of a professional nursing organization/organizations; Encourages others to consider nursing as a profession; Promotes health and wellness thru community activities
